2007-2008 Registration Forms

The following four forms are required for membership. Only the first two forms and a deposit check for your dues are required to establish your postmark date for the CLUB, INSTRUCTIONAL & FORSYTH PROGRAMS.
All fees received by Northlake Volleyball are fully refundable until 9 PM on Commitment/Registration Day, 11-14-07. After that date, $150 is retained by the club if a player withdraws before 12-31-07. After 12-31-07 there are no refunds for fees paid.

SRVA Membership Form - may be submitted on or before Registration Day.

USAV Medical Form - may be submitted on or before Registration Day.
This form must be notarized. We will have a notary on hand on Registration/Commitment Day, 11-14-07.
